Medical ethics, law and patient safety is 5% of our bank, 7 to 8 items on a 150-item paper. The SMLE blueprint lists patient safety, preventive medicine and ethics as a fifth block with no stated percentage, and rates breaking bad news, patient privacy, confidentiality, conflict of interest, sick leave, research ethics and social media practice at mastery level 3. The one legal fact we verified is the UAE Federal Decree-Law No. 4 of 2016 on Medical Liability. We did not retrieve its article contents, so do not rely on article numbers.
Weight and what this domain is
This is the smallest domain by weight and the one where clear thinking beats memory. Items present a scenario and ask what you should do. The answer follows a recognised principle (capacity, consent, confidentiality, duty to report, honesty, safety), applied to a Gulf setting where cultural and religious considerations are legitimate parts of the stem. Neither published blueprint gives an item count for this area. Qatar DHP lists a patient safety curriculum and an ethics handbook for residents among its references.
What is tested
| Cluster | Subtopics |
|---|---|
| Patient autonomy | Informed consent and capacity, confidentiality and permitted disclosure, breaking bad news and communication, end-of-life care, DNR and withdrawal of treatment |
| Law and duty | Medical liability and negligence in the Gulf, reporting duties (notifiable disease, suspected abuse, injury, death certification), sick leave certification |
| Safety | Medication safety and controlled drugs, blood transfusion and radiation safety, international patient safety goals, incident reporting and root-cause analysis |
| Professionalism | Impaired colleagues, whistleblowing and conflicts of interest, research ethics and social media conduct, cultural and religious considerations in care |
What we can state, and what we cannot
Verified
The UAE Federal Decree-Law No. 4 of 2016 Concerning Medical Liability was issued on 2 August 2016, came into force on 15 August 2016 and was published in Official Gazette No. 601. One amendment is recorded, last updated on the legislation site on 4 September 2023.
Not verified
Article-level content (consent, confidentiality exceptions, DNR, life support) was not retrieved, and the details of the 2023 amendment on abortion and consent are unverified. The same applies to other Gulf countries' laws. Do not cite article numbers, and do not rely on any source that quotes them without a link to the text. Where an item depends on a law, a well-written stem will state the rule it uses.
Principles safe to test everywhere
Capacity-based informed consent, confidentiality with limited statutory exceptions, a duty to report notifiable disease and suspected child abuse where the local rule requires it, honest disclosure of errors, and no certification of illness without examination. These follow international medical ethics, such as the WMA Declaration of Geneva.
The SMLE ethics list
| Mastery level | Topics named in the 2026 blueprint |
|---|---|
| 3 | Breaking bad news, patient privacy, confidentiality, conflict of interest, sick leave, research ethics, social media practice |
| 2 | DNR and DNI, abuse (spouse, child, elderly), abortion, medico-legal issues and whistleblowing, impaired physicians |
| 1 | Informed consent, discharge against medical advice, death certificate, fertility ethics, communication with peers' patients, second opinions |
The list is useful for all Gulf papers because it shows the SMLE's idea of what a general practitioner should do independently. Abortion, DNR and end-of-life items must follow the local legal framework, and we avoid article-level claims.

Culture, religion and communication
Ramadan fasting, consanguinity, modesty and family involvement in decisions are normal contexts in the region. The right answer respects them while keeping to the principles above. A patient's family may ask you to withhold a diagnosis; the principle is that the patient decides what they wish to know and who is told, and an item will test whether you ask the patient. Breaking bad news is a sequence: prepare the setting, find out what the patient already knows, ask how much they want to know, give the information in small pieces, respond to emotion, then agree a plan. Do not assume the typical patient's habits unless the stem says so.
Safety systems
Safety items test systems thinking. An error is an event to report and learn from, not a reason to hide it. Incident reporting and root-cause analysis look for contributing factors in processes and communication, not only the individual. Medication safety covers controlled drugs, look-alike names, dose checks and reconciliation at transitions of care. Transfusion safety turns on correct identification of the patient and the sample. Radiation safety turns on justification and keeping exposure as low as reasonably achievable. The international patient safety goals are a checklist: correct identification, effective communication, medication safety, safe surgery, infection prevention and fall prevention.
Three typical traps
- Answering from personal belief. A scenario about abortion, DNR or a family's wishes asks for the recognised professional response, not your own view. The right answer follows law, guideline and patient autonomy.
- Treating confidentiality as absolute. It has limited statutory exceptions, such as notifiable disease and suspected abuse. The trap goes both ways: disclosing too freely, or refusing a required report.
- Memorising legal detail. Article numbers and emirate-specific rules change and cannot be verified from our sources. Learn the principle and the reporting duty, and let the stem state the rule.
How to revise it
- Build a five-step routine: capacity, consent, confidentiality, duty to report, document. Apply it to every scenario.
- Learn the SMLE list in the table, with a one-line principle for each topic.
- Read the DHP-listed safety curriculum headlines and the international patient safety goals.
- Practise communication aloud: breaking bad news and explaining an error in two minutes.
- Do ten items a week, mixed into your usual sets, and review explanations for the principle, not the detail.
