Costa Rica in brief. Costa Rica regulates professions through colegios, professional associations with legal authority. The Colegio de Médicos y Cirujanos de Costa Rica runs a graduate competency exam for medical interns and graduates, and has pushed to extend an incorporation exam to foreign-trained specialists, a requirement that has been disputed in 2025 and 2026. Press reports have published pass rates by university. For nurses the Colegio de Enfermeras issues the title, with an exam that we could not verify. Other professions register by qualification. The language is Spanish.

Medical and health education in Costa Rica

The University of Costa Rica (UCR) in San José is the oldest and most selective medical school, and it has been joined by private schools, among them the Universidad Iberoamericana (UNIBE), the Universidad Latina and UCIMED. The degree in medicine and surgery takes about six years plus a supervised internship in the public Caja Costarricense de Seguro Social (CCSS) hospital system. Teaching is in Spanish, and public universities run the most demanding admissions.

Costa Rica also trains foreign students, and attracts many who study elsewhere in Latin America before returning to specialise. Nursing, pharmacy, dentistry and physical therapy are taught at both public and private universities and are accredited by the national accreditation council, SINAES.

The link between graduation and registration is the colegio. A graduate cannot practise without incorporating into the relevant professional college, and for medicine that means the Colegio de Médicos y Cirujanos. The college's exam is the clearest example of a profession policing its own gate.

The Colegio de Médicos competency exam

According to press reports in 2020 and 2026, the Colegio de Médicos y Cirujanos conducts an examination for medical graduates and interns, covering about 600 to 700 general practitioners per year. Reported pass rates varied sharply by university: 97% for UCR graduates against 46% for UIA graduates in the figures cited. The Colegio has also pushed for an incorporation exam for foreign-trained specialists, a proposal opposed by some groups as a barrier.

We could not verify the current structure of the exam: item numbers, passing standard, frequency and fee. The data we have is about the exam's existence and its results, which tells you something about difficulty and the weight of local preparation.

The status of the incorporation exam for foreign-trained specialists was disputed in 2025 and 2026. Check the Colegio's current notice before you apply or pay any fee.
  1. Confirm your category with the Colegio de Médicos: new graduate, intern, or foreign-trained specialist.
  2. Obtain your degree and verification, including recognition of foreign titles by the national universities' council (CONARE) if the degree is from abroad.
  3. Register for the next exam and request the syllabus.
  4. Sit the exam and, on passing, apply for incorporation.
  5. Complete the internship and obtain your professional licence.

Why the university makes a difference

The published pass-rate gap between schools has two possible readings. One is that the exam reveals differences in teaching quality. The other is that candidate selection differs: universities with stricter admission filter students before they begin. Either way, the practical lesson for a candidate is the same: do not depend on your school to prepare you for the format. Practise questions that mimic the exam, particularly in internal medicine, surgery, paediatrics, obstetrics and public health, and revise Costa Rican guidelines issued by the Ministry of Health and the CCSS.

  • Start question practice during the internship year.
  • Use CCSS clinical guidelines for common conditions such as hypertension, diabetes and dengue.
  • Keep a record of the weak areas that recur in your practice papers.

Nurses

The Colegio de Enfermeras de Costa Rica incorporates nurses and issues the professional title. Our data does not confirm an exam, and the entry is unverified. Foreign-trained nurses should ask the Colegio about degree recognition and any assessment. For nurses who plan to work in the United States, NCLEX-RN is a separate exam, covered on our United States page.

Midwives, pharmacists, dentists and physiotherapists

These professions register through their colleges and the Ministry of Health. Our data lists registration by qualification and no confirmed exam. Foreign graduates should expect recognition of the degree through CONARE and possibly a local assessment. As the medical colegio's exam shows, rules in this country can change, so verify them with the college itself.

Costs, timing and paperwork for foreign-trained doctors

If you trained abroad, two separate processes run in sequence. The first is recognition of your degree. In Costa Rica this is handled through the national council of rectors, CONARE, which compares your programme with local ones and may require supplementary exams or courses. The second is incorporation into the Colegio de Médicos, which sets its own conditions and, as the 2025 and 2026 disputes show, may change them.

  • Order apostilled copies of your degree and transcripts early.
  • Ask CONARE how long recognition currently takes and whether a programme review is needed.
  • Ask the Colegio whether you will be asked to sit the incorporation exam, and under which regulation.
  • Keep a copy of every notice and receipt, because disputes over requirements sometimes need evidence of what you were told.

Costa Rica's public health service, the CCSS, employs most doctors, and its own recruitment rules sit on top of the Colegio's. A job offer does not replace incorporation.

Language and recognition

The exam and the clinic are in Spanish. Foreign candidates need strong Spanish medical vocabulary. Recognition of foreign degrees runs through CONARE and then through the college, so build time into the plan for two separate processes. A Costa Rican licence is a domestic credential. Doctors who want to practise in the US still need ECFMG and USMLE, and the same logic holds for other destinations.
