Barbados has two medical sectors: UWI Cave Hill for local doctors, and offshore schools preparing for USMLE. Nurses take the regional RENR.
Barbados in brief. Barbados is where the offshore Caribbean medical school model is most visible. Our data lists eight offshore schools on the island, among them Ross University School of Medicine, the American University of Barbados and Queen's University College of Medicine, and their students sit USMLE Step 1 and Step 2. That is a US licensing story, not a Barbadian one. Locally, doctors register with the Medical Council of Barbados, and an exam for foreign graduates exists but is unverified. Nurses sit the Regional Examination for Nurse Registration (RENR), a CXC-managed MCQ exam.
Barbados has two distinct medical sectors. The first is the University of the West Indies Cave Hill campus, whose Faculty of Medical Sciences trains Barbadian doctors alongside the Queen Elizabeth Hospital in Bridgetown. The MBBS is a five-year degree in English, followed by an internship in public hospitals. The Barbados Community College and UWI prepare nurses, and UWI offers dentistry-related and pharmacy training in the wider region.
The second sector is the offshore schools. They are private, mostly US-owned, and their curriculum follows the US model: two years of basic sciences, then clinical rotations in the United States, Canada and elsewhere. Students come mainly from North America and the diaspora. They are not training for the Barbadian register at all; they train for ECFMG certification and the US residency match.
The link between graduation and licensing is therefore very different for each group. A UWI graduate completes internship and registers with the Medical Council of Barbados. An offshore graduate gets an MD that is licensable in the United States only after USMLE Steps 1, 2 CK and 3 and residency.
If you are studying at an offshore school in Barbados, treat this section as your plan, and use our United States page for the full exam details. The sequence is Step 1 (pass or fail), Step 2 CK (scored), and for most graduates Step 3 during residency. ECFMG certification requires passing the exams and meeting the school's eligibility, including that the school is listed in the World Directory of Medical Schools with the appropriate sponsor note.

The Medical Council of Barbados keeps the local register. UWI graduates register after internship. For foreign graduates, our data notes an exam that we could not verify. Offshore school graduates who want to practise on the island should not assume that an MD from a Barbados-based school gives a right to register in Barbados; write to the Council and ask.
The Barbados Nursing Council uses the RENR, a regional exam managed with the Caribbean Examinations Council. It has two papers of 100 to 120 multiple-choice items, about 200 in total, each lasting 2 hours 30 minutes, and sittings in April and October. The blueprint gives nursing practice 30%, clinical decision-making 20%, leadership and management 15%, professional conduct, health promotion and communication 10% each, and professional development 5%.
Participation varies by territory, and Barbados should be confirmed with the Council. Nurses who plan to work in the United States need NCLEX-RN, which is a separate exam with a different format; see the United States page.

These groups register through the Ministry of Health and their respective councils, and our research found no confirmed exam. Foreign-trained applicants should expect credential verification and possibly a local assessment. Write to the Ministry with your qualification and ask for the requirement in writing.
With a cluster of schools on a small island, the differences between them are large, and the marketing is similar. Ask for facts you can verify and write down the answers.
Be cautious about any school that cannot or will not publish its outcomes. A school that is confident of its results usually shares them.
Many UWI-trained Barbadians stay in the region, but the pull of the United Kingdom, Canada and the United States is strong. For a doctor leaving Barbados, the exam depends on the destination: USMLE for the US, PLAB or a recognised route for the UK, the Medical Council of Canada process for Canada. For nurses, NCLEX-RN or the relevant country's assessment applies. The RENR does not give access to those systems by itself.

English is the official language and the language of all the exams above. Barbadian Creole is heard in clinics, and clear standard English is expected in examinations. CAAM-HP accreditation of UWI programmes supports recognition across CARICOM, but each territory registers separately. For offshore students, recognition is global in principle through ECFMG and the US system, but residency competition is real, and the school's track record is worth checking before you commit.
