Ecuador's single habilitation exam for four health professions

Ecuador runs one computer-based CACES exam for doctors, nurses, midwives and dentists, and it applies to foreign-trained professionals once SENESCYT has registered their title.

Ecuador in brief. Four professions (doctors, nurses, midwives, dentists) must pass the CACES Examen de Habilitación para el Ejercicio Profesional, a computer-based multiple-choice exam, to practise. It applies to local graduates and to foreign-trained professionals once their title is registered with SENESCYT. Pharmacists and physiotherapists are, as far as our research shows, registered by qualification through SENESCYT and the health ministry's regulator ACESS, but check whether the habilitation exam has been extended to them. Midwifery here is Obstetricia. Spanish only.

Medical and health education in Ecuador

Medicine in Ecuador is a six-year degree, usually five years of study followed by an internship (internado rotativo), taught in Spanish at around twenty universities. The Council for Quality Assurance in Higher Education (CACES) evaluates programmes, and SENESCYT registers the graduate’s title. Nursing and dentistry run four to five years, and pharmacy (bioquímica y farmacia) about five.

PharmacyAbout 5 yearsRegistration
Nursing4 yearsRural year, registration

After graduation, doctors do a year of rural service (año de salud rural) with the Ministry of Public Health. Practice requires title registration with SENESCYT and registration with the health quality authority (ACESS). The final gate is the CACES habilitation exam (Examen de Habilitación para el Ejercicio Profesional), a computer-based multiple-choice test required of both local and foreign-trained doctors; check the current CACES notice for dates and format. Specialty training is competitive and ranked. Foreign-trained doctors need their degree recognised through SENESCYT before registering. Confirm each step with the Ministry.

One exam, four professions, one regulator of quality

Ecuador's model is simple on paper. The Consejo de Aseguramiento de la Calidad de la Educación Superior (CACES) runs a single habilitation exam framework, and the health careers that fall under it share the same computer-based, multiple-choice format. Where other countries have a separate board for each profession, Ecuador has one quality-assurance body setting the rule for all of them.

What makes it distinctive is the dual audience. The exam is not only for Ecuadorian graduates. A foreign-trained doctor or nurse who has registered their title with SENESCYT, the higher-education secretariat, is also required to sit it. This is why the CACES exam features among the exams that Venezuelan, Colombian and Peruvian emigrants consider alongside EUNACOM and ENAM.

Doctors, nurses, midwives and dentists: the CACES exam

The Examen de Habilitación para el Ejercicio Profesional is computer-based and multiple-choice. We have not verified the current number of items, time limit, pass mark or sitting calendar, so take those from the CACES notice for the cycle you plan to enter. The same exam is required to practise in Ecuador, which makes it a gate rather than a ranking exercise.

Midwives should note the local title. Obstetricia is the career name, and obstetrices are the practitioners. Search for that term, not "partera" or "midwife", when you look for programmes and forms.

  1. Register your title with SENESCYT. A foreign title needs legalisation first.
  2. Check the CACES calendar for the next habilitation sitting for your career.
  3. Enrol for the exam through the CACES platform and keep your confirmation.
  4. Sit the computer-based test at the assigned venue.
  5. Take the result to the health authority to register for practice and apply for posts.

Pharmacists and physiotherapists

In our data these two professions register by qualification through SENESCYT and ACESS (the Agencia de Aseguramiento de la Calidad de los Servicios de Salud y Medicina Prepagada), under the Ministerio de Salud Pública. This may change. If CACES extends habilitation to more careers, pharmacists and physiotherapists would be among the likely candidates, so treat this section as provisional.

Language and recognition

Spanish only. The recognition step that precedes the exam is SENESCYT registration of the foreign title. Documents must be legalised or apostilled and, if necessary, translated. SENESCYT may decline to register a title from an unrecognised institution, in which case the exam is out of reach until that is resolved. Start the registration before anything else.

Ecuador also has a rural-service tradition for new doctors, and posts in the public system are filled through national processes. Ask the Ministerio de Salud Pública how foreign graduates fit into those processes, as rules have changed more than once.

Preparing for a habilitation exam

A habilitation exam differs from a ranking exam in one important way: your goal is to clear the bar, not to beat other candidates. That changes how you should study. Rather than chasing rare topics, make sure you are reliable on the common conditions and procedures in your own career, because those make up most of what a basic-competence exam tests. For a doctor that means primary care and emergency medicine, maternal and child health, and infectious disease. For a nurse it means fundamentals of care, medication safety, maternal and neonatal care, and public health programmes.

Because the exam is computer-based, practise on a screen. Get used to flagging questions, reviewing them and managing the clock. Candidates who studied mostly from paper notes sometimes find the interface a distraction on the day.

  • Look up the Ecuadorian clinical practice guides (guías de práctica clínica) from the Ministerio de Salud Pública.
  • Learn the national vaccination schedule and the main public health programmes.
  • Confirm the blueprint or topic list on the CACES notice, if one is published for your career.

Why Ecuador matters for Venezuelan and Colombian candidates

Professionals leaving Venezuela and Colombia often compare three exams side by side: EUNACOM in Chile, ENAM in Peru and CACES in Ecuador. The Ecuadorian route has one significant advantage on paper, which is a single exam for four professions. It also has a disadvantage: it depends on SENESCYT approving the title first. A candidate who plans around the exam date but not around the registration timeline can lose months. Start with the document that takes longest, which is usually the diploma legalisation in the country of origin.

Where to check the rules

CACES publishes the habilitation calendar and rules, SENESCYT handles titles and the Ministerio de Salud Pública handles practice registration. TriagePass is independent of them.
