Bulgarian graduates sit a university state exam and then register with their professional union. Foreign graduates face recognition, a terminology exam and separate admission steps.

Bulgaria in short. There is no national licensing MCQ. The gate is the state exam (държавен изпит) at the end of the degree, set and marked by your own medical university, followed by registration with the professional body.
Doctors register with the Bulgarian Medical Union (Български лекарски съюз, BLS). Pharmacists and dentists register with their own unions. Nurses, midwives and physiotherapists register by qualification.
The language of the state exam is Bulgarian for Bulgarian-track students and English for the English-language programmes. Graduates from outside the EU who want to practise in Bulgaria face a Bulgarian terminology exam and a separate admission procedure.
Medicine is a six-year single-cycle degree. Dentistry and pharmacy run for five years. Nursing, midwifery and physiotherapy are professional bachelor degrees of about four years. The medical universities in Sofia, Plovdiv, Varna and Pleven carry most of the load, and Trakia University in Stara Zagora and Sofia University also teach medicine.
Bulgaria is one of the main European destinations for international medical students. Several universities run full English-language MD programmes with their own admission tests. Those students study alongside the Bulgarian-language cohort, but the two groups sit separate state exams in their own language of instruction.
Graduation and registration are linked but not the same thing. The state exam is part of the degree, so passing it gives you the diploma. Registration with the BLS is the step that lets a doctor practise. Specialisation is a separate competition run by the Ministry of Health, and a doctor who wants a residency place must win a position there.
What follows graduation depends on where the graduate wants to work. Home-country exams are the real next hurdle for most international graduates: PLAB for the UK, the AMC exam for Australia, USMLE for the US, the Prometric and Dataflow route for the Gulf, the NMC screening test for India and the national registration exam for Pakistan. Check each regulator's current rules before you commit, because accepted schools and eligibility change.
For locally trained graduates the answer is simple. The only exam between you and registration is the university state exam. There is no extra national paper after it. A graduate presents the diploma, and registers with the relevant union or association.
Foreign-trained candidates split into two groups. EU, EEA and Swiss nationals use the recognition system under Directive 2005/36/EC. Everyone else goes through national procedures: academic recognition of the diploma by NACID (the national information centre), professional recognition by the Ministry of Health, and a Bulgarian-language requirement.
The directive's rights belong to EU, EEA and Swiss nationals. A non-EU national who holds a Bulgarian diploma should not assume other EU countries will recognise it automatically. Each destination country applies its own rules to that case, so ask the regulator before you rely on it.
The medical state exam is organised by each university under national requirements. It combines a written part with a practical or oral part, and the format differs between Sofia, Plovdiv, Varna and Pleven. Treat any "standard format" you read online as a guide only and take the exact structure from your own faculty.
After the diploma, a doctor joins the BLS and, in time, applies for specialisation. The specialty entry process is a competition. It includes a written test, so this is where an MCQ-style preparation becomes relevant for Bulgarian graduates.
A route exists for non-EU doctors who need to be admitted to Bulgarian practice. As described in the government's guidance for Ukrainian doctors, the Ministry of Health admits the candidate to the exam, the candidate takes a Bulgarian professional-terminology exam, and then takes the state exams at a Bulgarian medical university. Read the current guidance on the government portal, because it was written for a specific group and the details may differ for other nationalities.

Pharmacists complete a five-year Master. The final state exam has a written element and a practical or oral element, and the university sets it. Registration then goes through the Bulgarian Pharmaceutical Union.
Dentists follow the same pattern with a state exam at the end of the dentistry degree and registration with the Bulgarian Dental Association. Non-EU dentists and pharmacists go through NACID recognition first.
These professions do not have a separate licensing exam. The final state exam sits inside the bachelor degree, and registration with the Bulgarian Association of Health Care Professionals (for nurses and midwives) follows by qualification. Physiotherapists register through the Ministry of Health and their professional association. Non-EU graduates need NACID recognition, a possible adaptation period and Bulgarian.
| Profession | Gate to practise | Set by | Language |
|---|---|---|---|
| Doctor | University state exam, then BLS registration | Medical university | Bulgarian or English (by programme) |
| Doctor, specialty entry | Written test in a competition | Ministry of Health | Bulgarian |
| Pharmacist | University state exam, then union registration | University | Bulgarian or English |
| Dentist | University state exam, then association registration | University | Bulgarian or English |
| Nurse and midwife | Degree state exam, registration by qualification | University | Bulgarian |
| Physiotherapist | Degree state exam, registration by qualification | University | Bulgarian |
Bulgarian is the working language of the health system. Non-EU graduates who want to treat patients in Bulgaria must show professional-level Bulgarian, and the terminology exam for doctors is administered through the Ministry of Education. English-taught degrees do not remove that requirement once you leave the university setting.
For work in other EU countries, a Bulgarian diploma in medicine, dentistry, pharmacy, nursing or midwifery is covered by automatic recognition for EU nationals, provided the programme meets the directive's minimum training requirements. Physiotherapy is handled under the general system, so the destination country may ask for an aptitude test or adaptation period.
Unverified detail: some profession-specific rules above come from secondary summaries. Confirm the current requirement with the Ministry of Health, the relevant union and your university before relying on it.

Start with the exit exam you will need after Bulgaria, not just the state exam. A student who plans to work in the UK should check PLAB and the GMC's accepted qualifications in the third year, not the sixth. A student aiming for the US should check the ECFMG's current accreditation requirement for the school. A student heading to the Gulf should start Dataflow primary-source verification early, because it takes weeks.
Use the Bulgarian state exam as a base for revision. Its clinical subjects overlap with most international MCQ exams, but the item style does not. Practise in the format of the exam you will actually sit.
No. Doctors, dentists and pharmacists sit a state exam set by their own university at the end of the degree, then register with the relevant professional union. Nurses, midwives and physiotherapists register by qualification.
The state exam follows the programme's language of instruction, so English-track students are examined in English by their university. Check your faculty's regulations for the exact format.
The Bulgarian Medical Union (Български лекарски съюз, BLS) is the professional body doctors register with in order to practise.
Through a competition run by the Ministry of Health that includes a written test. Check the current subject list and rules with the Ministry.
For EU, EEA and Swiss nationals, medicine, dentistry, pharmacy, nursing and midwifery diplomas are recognised under Directive 2005/36/EC if the programme meets the minimum training standards. Non-EU nationals should ask the destination regulator, because the directive's rights belong to EU nationals.
Yes. The admission route for non-EU doctors includes a Bulgarian professional-terminology exam before the state exams at a Bulgarian medical university.
