Bahrain makes the licensure exam compulsory unless the Supreme Council of Health approves an exemption (NHRA Decision No. 29 of 2020). The exam is delivered by Prometric with a 3-hour seat and 2.5 hours of exam time; the pass mark is 60%. The attempt rule is a ladder: four consecutive attempts within three years of the first, at least six months of retraining after a fourth failure, and then two more attempts within two years. NHRA does not publish the question count or the fee. Checked 21 October 2026.
The rule that shapes everything: pass or do not practise
In some Gulf systems the exam is waived for most experienced applicants. Bahrain goes the other way. NHRA Decision No. 29 of 2020 requires licence applicants in the covered professions to pass the licensure exam. The only exception it states is an exemption approved by the Supreme Council of Health. We found no default exemption for experienced foreign doctors in the text we reviewed, and no automatic recognition of another Gulf licence. For a doctor, that makes the attempt rules more important here than anywhere else.
The attempt ladder
| Stage | Attempts | Condition |
|---|---|---|
| 1. First window | Attempt 1, then up to three more (attempts 2 to 4) | All four within 3 years of the first attempt |
| 2. After a fourth failure | None until retraining is done | At least 6 months of retraining |
| 3. Second window | Two more attempts (5 and 6) | Within 2 years |
On this reading the total is six attempts, spread across a long period. The decision text sets the exact wording and the start point for the second window, and later amendments may change it. Read Decision No. 29 yourself before relying on our summary. The sources we reviewed do not define what counts as retraining, who certifies it or what evidence NHRA wants. That is a question for NHRA, not something to guess.
A worked timeline
Dates make the ladder concrete. Suppose your first attempt is on 1 March 2027.
- Attempts 2, 3 and 4 must all fall on or before 1 March 2030, three years after the first.
- If you fail a fourth time on, say, 15 January 2028, you are not allowed to sit again until at least 6 months of retraining are complete, which is no earlier than mid-July 2028.
- You then have two further attempts within two years of the point the decision text specifies.
The dates are illustrative. The lesson is that the three-year window runs from the first attempt, not from the last, so a series of quick, under-prepared attempts uses up the window as well as the number. Spacing your attempts to leave time between them is the better use of the ladder.

Format: what Prometric says
The Prometric NHRA page lists the physician licensure exam with a 3-hour seating time and 2.5 hours of exam time. It does not explain the half hour of difference and does not state a question count or an option count. We therefore do not give either. If the paper were 150 items, 2.5 hours would be 60 seconds an item. We do not know that it is, but training at 60 seconds an item is the conservative choice, and you lose nothing by being faster than you need. Pass mark: 60% for all Bahrain licensure exams.
Booking
- Apply to NHRA. The contact listed is licensure@nhra.bh, and NHRA's application and licensing procedure page describes the process.
- Receive your eligibility number. It is valid for up to 4 months.
- Register with Prometric inside that window and book a slot.
- Sit the exam and read the result as NHRA and Prometric instruct.
The fee is not published on the pages we checked. The four-month window matters as a deadline: book early enough to leave a spare month for illness or a centre closure, but do not book so early that you sit unready. Because the attempt ladder starts from your first attempt, the first booking is the one that deserves the most care.
Exam-day pace when the count is unknown
Because the question count is unpublished, adopt a pace that works for either a short or a long paper. Check the number of items on the first screen if it is shown, divide your 150 minutes by it, and set your checkpoints on the spot. If no count is visible, hold 60 seconds per item and review your progress every 30 minutes. Flag and move on rather than stall. The two-and-a-half hours are exam time, so a stalled item costs the same minutes whatever the count.
Verification: DataFlow and NHRA
DataFlow has been NHRA's PSV partner since 2017. Its page says applicants can verify their NHRA Good Standing Certificate at no extra cost, that an existing DataFlow report can be transferred to NHRA, with the transfer ready within 5 days if no new documents are submitted, and that applicants pay NHRA's fees through DataFlow. No amount or standard turnaround is given. See the DataFlow guide.
Retraining: what we know and what to ask
The decision text we reviewed places a minimum of six months of retraining between the fourth failure and the next attempt. The pages we checked do not say what the retraining must consist of, where it can be done, who supervises it or what proof NHRA expects. These matter, because the six months are only the minimum. Ask NHRA in writing before you need the answer: what qualifies as retraining, whether a clinical post counts, who certifies it, and how the evidence reaches NHRA. A candidate who learns the requirement only after the fourth fail loses time that a prior enquiry would have saved.
A preparation plan shaped by the ladder
The ladder rewards a single, serious first preparation and a deliberate repair plan if it fails. A sensible structure follows.
| Stage | Preparation | Readiness check before booking |
|---|---|---|
| Attempt 1 | 12 to 16 weeks on the study plan, with three full mocks | Two consecutive mocks within five points of each other and above your target |
| After a fail (attempts 2 to 4) | 6 to 8 weeks of repair: error log first, weak domains second, full mocks last | The domain that cost you most is above 60% in timed sets |
| After a fourth fail | Retraining as NHRA defines it, plus a fresh 12-week plan | A new baseline, because the earlier plan did not work |
These durations and checks are our recommendations. NHRA publishes none of them. Their purpose is to stop you spending attempts as practice sittings.
How Bahrain compares
Bahrain has no default exemption, a bounded number of attempts inside a time window, and a mandatory retraining step. Qatar allows five consecutive attempts without a stated gap, the UAE sources describe three, and Oman currently states no maximum. Among the regulators that state a limit, Bahrain's ladder is the only one that forces a pause for retraining, which makes a careful first attempt more valuable here than anywhere else. The comparison guide shows all seven on one page.
Points candidates often misread
- "Four attempts, then retrain" is not "four attempts in total." The decision text we reviewed adds two more attempts after retraining, which is why we read the total as six. Check the current wording.
- The three years run from the first attempt. They do not restart with each failure.
- An eligibility number is not a booking. It is valid for up to four months, and you still have to register and book with Prometric inside that period.
- A good-standing certificate is a document, not an exemption. DataFlow verifies it for NHRA at no extra cost, but verifying it does not waive the exam.
- Passing the exam is not the licence. NHRA's licensing decision and your employer's steps follow, and we found no published timeline for them.
Using the ladder in your strategy
- Treat attempt one as the attempt. It starts the three-year clock and uses one of four places in the first window.
- Space your retakes. After a fail, rebuild from your error log. Prometric does not show you a domain breakdown in the sources we checked, so your own mock data is your feedback. See the study plan and the scoring guide.
- Do not count on the second window. It needs retraining, whose definition we could not find, and it adds months to your timeline.
- Ask about exemptions early. If you hold a qualification you think should qualify, ask NHRA about the Supreme Council of Health route before you book, because that route is the only exemption the decision states.
What is not published
Question count, option count, content weights, the exam fee and the definition of retraining are not published on the pages we checked. The same is true for pass rates. Plan with what is stated, ask NHRA for the rest, and keep the replies. Write down the date of every attempt, the centre, the booking reference and any correspondence. If a dispute about how many attempts you have used ever arises, your own record is what resolves it, and the three-year window makes exact dates matter. The comparison guide shows how Bahrain's ladder sits against the other six regulators.
